Unity Health Network named top healthcare provider in Anthem’s Medicare Advantage network

Unity Health Network was recently named as one of only 19 national recipients in Anthem Blue Cross and Blue Shield’s inaugural Care Provider Recognition Program. Created in partnership with its affiliated health plans across the country, Anthem’s new program celebrates top performing independent and hospital-owned medical groups across the nation, recognizing those who deliver unparalleled patient care. The program is part of Anthem’s commitment to collaboration and active partnership with care providers to create a positive impact on the health of plan members.
Unity Health Network is among less than 5% of care providers in Anthem’s Medicare Advantage network who qualified for the Care Provider Recognition Program. Unity was selected based on key health measures for care delivered to Anthem’s Medicare Advantage members. Measures include preventive care screenings, immunization rates, chronic disease management, medication adherence, and several other health outcomes metrics.
Unity Health Network has focused its efforts on comprehensive care initiatives that have yielded extremely positive results in achieving improved outcomes and lowered costs of care. Initiatives such as Unity’s performance program, high-risk care management team, emphasis on data analytics, and alignment with hospitals, skilled nursing facilities and home health agencies have also contributed to the recognition. Through deliberate actions, Unity Health Network has been able to lower hospital admissions, reduce readmissions, improve diabetic scores, and improve cancer screening rates, in addition to a number of other gains.
